PMO strikes balance for medical education regulator

NEW DELHI: Prime Minister’s Office has finetuned the solution of right regulator for medical education. It has been decided that while health ministry’s National Council for Human Resource in Health (NCHRH) will be the sole regulator for medical education — including all degress like MBBS and MD — the human resource development ministry’s National Commission for Higher Education & Research (NCHER) will have a say on medical as well as inter-disciplinary research.

NCHRH will be the sole custodian of MBBS courses throughout the country, including syllabus, quality and number of seats in each institute. A highly placed source in the HRD ministry said, “A solution has been found.”

Former Union health secretary K Sujatha Rao told TOI, “We will set the standards of medical education and regulate the norms just like we do now. However, universities will now have the freedom to innovate and be creative in their teaching and start new diverse courses if they like.”

HRD and health ministries have been told to rework their respective bills on NCHER and NCHRH.

Republicans pitch malpractice reform in MT

HELENA, Mont. – Republicans said Tuesday that proposals to limit medical malpractice claims are among their ideas to lower health care costs as they try to undermine implementation of federal health care reform.

One proposed bill aims to give doctors more protection from lawsuits. Another would limit the amount survivors could seek if a relative died due to medical malpractice.

Critics countered that the proposals would not reduce costs and would only hurt injured patients seeking relief.

The health care debate has been brewing all session with partisan fights over a long list of proposals.

In general, Republicans say they want more free market reforms aimed at lowering costs as an alternative to the federal health care law. Democrats argue that the Republican proposals will do almost nothing to lower costs, and that GOP efforts to undermine federal law could be unconstitutional in some cases and ineffective in others.

The health care debate played out Tuesday in several different places at the Legislature.

In the morning, Republican-backed bills took on the issue of medical malpractice costs.

Many doctors and medical groups supported plans that attempt to give them relief from medical malpractice insurance costs or claims. The doctors said they routinely order unnecessary tests simply out of fear of lawsuits.

Opponents led by trial lawyers argued that the doctors only suffer malpractice claims when something wrong is done. They said doctors have freedom now to order only the tests and procedures they feel are needed, and only engage in defensive medicine out of an irrational fear of being sued.

Republican leaders point to other measures as ways to reduce costs.

One would let consumers buy out-of-state health insurance to increase lower-cost options, a move regulators counter gives Montanans no recourse if the company abandons them.

Other plans aim to encourage cheaper high-deductible insurance plans, and give more tax relief for health expenses.

House Republicans, meanwhile, kept pushing back against the federal health care law.

The House voted down party lines 68-32 to amend the Montana Constitution to say no Montana should be penalized “for exercising the right to choose whether to purchase or participate in health insurance coverage.”

“As Republicans we have a goal that we want patient-driven health care for our constituents. And we want that health care driven by the markets,” said House Majority Leader Tom McGillvray of Billings.

Democrats bristled at the suggestion that Montana can circumvent federal law and the U.S. Constitution simply by changing its own constitution.

“We are sent here to respect the law, not to try and undermine it for political reasons,” said Rep. Pat Noonan, D-Butte.

The proposed change to the state constitution faces little chance of appearing on the ballot for voters. Such proposal need two-thirds support from the Legislature, and Republicans alone don’t have enough votes to get to the 100 votes overall needed after a Senate vote.

Another Republican proposal would ban the state government from setting up the insurance exchanges called for by the federal law. The state insurance commissioner seeking to establish the exchange has argued the federal government will set one up for Montana if the state doesn’t.

Sedona City Council OK’s Medical Marijuana Ordinance

The Sedona City Council became the first Verde Valley community to approve a local Medical Marijuana Ordinance, Feb. 8. While the state initiative was approved by Arizona voters during the last election, local governments are allowed to regulate where the drug may be sold in their communities.

The approved Sedona ordinance allows medical marijuana dispensaries and their associated cultivation sites within the Sedona City limits in the C-1, C-2 and C-3 commercial zoning districts, and only if these facilities are located at least 500 feet from a school, church, park, recreational facility, day care, library or youth center.

Under Proposition 203, the Arizona Medical Marijuana Act, which was approved by voters in November of 2010, the Arizona Department of Health Services (ADHS) has until the end of March 2011 to adopt and enforce a regulatory system for the distribution of marijuana for medical use. The act places an initial limit of approximately 125 nonprofit medical marijuana dispensaries statewide, corresponding to one per every 10 pharmacies. Each county is also allowed at least one dispensary. Based on recent information from ADHS, it is likely that one registration certificate for a medical marijuana dispensary will be issued in the Sedona area this summer.

Other Verde governments are expected to approve similar ordinances. In Cottonwood, the City’s Planning Commission is getting its first look at a proposed zoning ordinance amendment during the Planning and Zoning meeting Monday, Feb. 28 at 6 p.m. The proposed ordinance amendment establishes performance standards for related uses and eligible zoning districts for dispensaries, according to the department.

Parents often think over if their children with anxiety will outgrow their anxiety or if they will get worse over time. Some anxious children, if inspired to face their fears, develop contending skills on their own as they mature. In that sense, they do surpass their fears. On the other hand, children with anxiety that avoid their fears can become quite disabled by them. With prolonged aversion of age-appropriate activities, important skills are lost and the children with anxiety become increasingly symbiotic on the few people to whom they relate. Often, family conflicts result due to poor coping skills. With increased family conflict and decreased ability to keep up at school and with peers, their self-esteem plummets.

Fortunately, most children with anxiety can outgrow their fears by gradually facing them. Each time a feared situation is faced, the child becomes a little less responsive to it. After a child is repeatedly exposed to the feared situation and is helped to successfully master his or her anxiety, the fight-or-flight reaction to that situation will dilute.

Over a lifetime, anxiety tends to come and go in those people who are predisposed to it. Exacerbations tend to happen when supports are lost or a new stressor is faced. For example, the expectation for children to behave more self-supportingly at adolescence can be difficult for a sensitive child who has previously relied a great deal on parental support. However, for a child that has learned adjusting skills, this difficulty is not a cause for despair. Instead, it is a signal that previously learned coping skills need to be reviewed, and a few new ones may need to be added. Each time a new situation is mastered, confidence builds, allowing the child to face new challenges and live a productive, fulfilling life.

Deciding on the best method for inducing your child with anxiety to face what is feared may require some thought. As a rule, whatever works and does not harm is best. If your child lacks the skills needed to face what is feared, some training by a psychologist in those skills may be needed before exposing the child to what is feared. For example, a chronically shy child that for years has not talked with companions may lack the social skills to do so. Once the necessary skills have been taught, your child must face what is feared through continuous exposures to it.
